WAIVER
I, the undersigned, being the parent/legal guardian of the above-named participant, or if I am the participant and of legal age, hereby agree with Rondo Time LLC, doing business as BE PRO Futbol and BE PRO Scouting (referred to in this waiver as “BE PRO Futbol”), as follows:
Assumption of Risk:
I acknowledge that participation in soccer events and training involves inherent risks, including the risk of physical injury, illness, or other harm. I voluntarily assume all risks associated with participation in BE PRO Futbol events and training sessions.
Release and Waiver of Liability:
I release and discharge BE PRO Futbol, its officers, directors, coaches, agents, and employees from any and all claims, liabilities, or demands that arise from or are in any way connected with participation in any BE PRO Futbol event or training session, whether caused by negligence or otherwise, to the fullest extent permitted by law.
I FURTHER UNDERSTAND THAT SPORTS INVOLVE PHYSICAL CONTACT BETWEEN PLAYERS, THAT SERIOUS ACCIDENTS OCCASIONALLY OCCUR DURING SUCH SPORTING ACTIVITIES, AND THAT PARTICIPANTS IN SUCH SPORTING ACTIVITIES OCCASIONALLY SUSTAIN SERIOUS PERSONAL INJURIES (INCLUDING DEATH) AND/OR PROPERTY DAMAGE, AS A CONSEQUENCE THEREOF. KNOWING THE RISKS OF PARTICIPATION, NEVERTHELESS, I HEREBY AGREE AND I ASSUME THOSE RISKS AND RELEASE AND HOLD HARMLESS RONDO TIME LLC, DBA BE PRO FUTBOL AND BE PRO SCOUTING, AND THEIR AGENTS, SPONSORS AND EMPLOYEES WHO (THROUGH NEGLIGENCE OR CARELESSNESS) MIGHT OTHERWISE BE LIABLE TO ME, (OR OUR HEIRS OR ASSIGNS) FOR DAMAGES.
I attest that I am physically fit and have no known medical conditions which prohibit participation in this sport. I agree to follow all laws, rules and guidelines regulating the conduct of the league, camp or clinic. I understand and agree I are responsible for the mechanical and/or operating condition of any and all sporting equipment provided by me for my use, and I agree that I will continuously inspect and maintain all equipment used, even if we have obtained any of the equipment from Rondo Time LLC (BE PRO Futbol), their agents, sponsors and/or employees.
I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY FOR MYSELF AND MY CHILD AND A CONTRACT BETWEEN MYSELF, MY CHILD AND RONDO TIME LLC, DBA BE PRO FUTBOL AND BE PRO SCOUTING, AND THEIR AGENTS, SPONSORS AND EMPLOYEES, AND I HAVE SIGNED IT OF MY OWN FREE WILL.
Use of Image and Likeness:
I grant BE PRO Futbol and its representatives the right to take photographs, video, and other recordings of me (or my child, if applicable) during events and training sessions. I authorize BE PRO Futbol to use, publish, and distribute these images and recordings in any promotional materials, websites, social media, or other media formats without compensation or prior approval. I understand that these images may be used to promote BE PRO Futbol’s activities and programs.
Indemnification:
I agree to indemnify and hold harmless BE PRO Futbol, its officers, directors, coaches, agents, and employees from any claims, actions, or damages arising out of participation in BE PRO Futbol activities, including reasonable attorneys’ fees and costs.
By signing below, I acknowledge that I have read, understood, and voluntarily agree to this waiver and release of liability.
